Immunizations

• 5 doses of Dtap
• 4 doses of Polio
• 2 doses of MMR
• 3 doses of Hepatitis B
• 2 doses of Varicella 
• 4 doses of Pneumococcal
• 1 booster dose of Tdap required for seventh graders
• 1 Meningococcal conjugate vaccine (MCV) for seventh graders
• 1 booster dose of Meningococcal conjugate vaccine (MCV) for all twelfth graders **Effective 2020-2021 school year

Effective July 1, 2016 - all students who are enrolling in a public school in N.C. for the first time must present a health assessment within 30 days of enrollment. This applies to all students in grades Kindergarten - 12.  

All health assessments must be completed by a licensed health care provider on this form. 

In the event of an outbreak of a vaccine preventable disease - Students who have not provided proof of full vaccination due to medical exemption or religious beliefs for all state required vaccine preventable illnesses (Measles, Pertussis, Mumps, etc.) will be excluded from school based upon recommendations from the local Health Department and the CDC.  

Nurses and First Responders are typically available during school hours to assist with student health needs. It is the parent's responsibility to communicate health needs to the coordinator of any sports or other extracurricular activities that occur outside of the school day.  Parents should also ensure that students have access to any emergency medications that are required outside of school hours.
