Student Enrollment Information

The following forms are required to complete the enrollment process.  A withdrawal form from the student’s previous school is helpful.  The child’s birth certificate and immunization record is required within 30 calendar days of enrollment.  Parents may be asked to complete additional forms when they present their child for enrollment at their designated school.
