Open Enrollment for All Other Benefits from April 2 - June 7

This is to announce open enrollment for all other benefits except the medical, which concluded Friday, March 30th. Open enrollment dates are from April 2 – June 7. Pam and/or Stephanie Leonard will be making visits to your location as we have done in several years past, to discuss and facilitate the changes you may need. To view the brochure, click here.

Once again this year, you should have received your confirmation statement that is an overview of your benefits. Also in that same envelope, you have received a brochure explaining your options and a schedule of Mrs. Leonard’s visit dates. Because there are changes to your benefits, everyone needs to be seen to discuss the changes. Please see the appointed contact at your location for scheduling an appointment.

Please be mindful that June 7th is the last day to make changes. All forms must be turned in by that day.

If you are requesting any changes to your benefits, please download the universal form and/or the flexible spending account form, print and complete it, and turn it in to Mrs. Leonard during your appointment with her.
